Lakeland, FL, serving patients in Polk County. Your Role as a
Physical Therapist As a Physical Therapist with Concierge Home
Care, you will deliver personalized, patient-centered care while
leveraging advanced technology and AI-supported tools designed to
streamline documentation and improve efficiency. Key
Responsibilities: Travel to patients’ homes to provide
individualized physical therapy services that promote recovery,
mobility, and independence Conduct comprehensive evaluations to
assess patient conditions and rehabilitation needs Develop and
implement goal-oriented treatment plans in collaboration with
patients, caregivers, and the interdisciplinary care team Utilize
AI-assisted documentation and charting tools to support timely,
accurate note completion and faster turnaround times Guide and
supervise Physical Therapy Assistants to ensure high-quality,
compliant care delivery Our technology-forward approach allows you
to spend more quality time with patients and less time charting,
supporting better work-life balance. Qualifications Unencumbered
Florida Physical Therapy License(required) Valid driver’s license,
auto insurance, and reliable transportation (required) Experience
with OASIS documentation(preferred) CPR Certification(required)
Ability to delegate and supervise Physical Therapy Assistants
