Duties

The duties of Care management services include but are not limited to the following:

The RN will demonstrate a therapeutic relationship with the acute and chronic mentally ill patient by focusing attention on the emotional and behavioral concerns through a mutually defined professional relationship. The RN performs structured clinical assessments and complex, systematic psychiatric assessments which include the total health status of the patient. This requires knowledge of medical terminology, diagnostic categories, and ability to perform chart reviews. The incumbent begins the coordination process at the time the patient is identified and works collaboratively with staff
within the Mental Health Clinics, as well as staff throughout the Veterans Health Administration (VHA) facility that may also be involved in his/her care.
The RN reviews assessments completed by others, and obtains missing information from providers, patients and medical records. The RN will demonstrate leadership in utilizing verbal and behavioral treatment modalities based on the Recovery Model of care. The RN will initiate patient and family education to include information about mental illness, medication, benefits and consequences of behaviors, coping strategies, evaluations and assessments and legal treatment modalities. This RN will also administer long acting psychiatric injections. This includes collaboration in the development of an appropriate treatment plan and/or safety plan for patients, and the ability to communicate that plan to the patient and family. Provides nursing intervention as needed and as defined by the RN Scope of Practice in the management of medication and other health issues.
The outpatient mental health RN will demonstrate extensive oral and written communication skills: contacts patients through written correspondence and telephone calls, and conducts follow-up interviews and creates and maintains forms, surveys, instruments, and materials for proper coordination of the different treatment modules available. The RN may facilitate recovery-based psycho educational groups. The RN will demonstrate appropriate use of a computer.
The outpatient mental health RN must be able to fully participate in Part 1 and Part 2 of Preventive Management of Disruptive Behavior (PMDB) requirements and utilize therapeutic communication skills in managing care. Maintaining a current BLS certification is required
Provides crisis intervention services which include a nursing assessment of needs and safety concerns and then linking the veteran as needed to the proper services for appropriate intervention to ensure safety. Provides comprehensive health triage both on the phone and face to face including but not limited to assessing walkins to include an evaluation of safety and treatment needs as indicated with goal of meeting the needs of the Veteran in a timely manner. Provides communication and collaboration with families and /or other concerned individuals as appropriate (given confidentiality concerns) to support the Veterans Recovery goals. Responding to My HealthyVet secure messages and/ or triaging secure messages to providers. Triaging mental health consults either and/or assisting with the appropriate referral to services.
RNs will co-facilitate therapy and/or orientation groups. RN will collaborate with other disciplines and caregivers and uses group processes for decision making in interdisciplinary problem solving. RN will share clinical/professional expertise with others within the facility or beyond through consultation,
presentations, publications, participation in professional organizations, etc. as a resource person or consultant in an area of practice RNs will assist with the process of voluntary and involuntary hospitalization including providing one-to-one supervision of individuals requiring such service.
RN will assist with patient care by completing CPRS Mental Health Reminders and by completing patients' mental health treatment plans.
PLEASE NOTE: All RN positions require Basic Life Support (BLS). If selected, Applicant will provide proof of current certification prior to employment. Only BLS certification which included Automated External Defibrillator (AED) obtained from either the American Heart Association (AHA) or the Military Treatment Training Network (MTN) will be accepted.
